ERR Exception in GameActionTicker

ERR Exception in GameActionTicker
Object reference not set to an instance of object.
This error appears often and it causes many bugs such as mobs don't die, spells can't be casted etc.
Is there any way to solve this problem?

Heal is fixed and speed decreaes I'm not sure. There are cases when OnEffectExpires is called twice but I'm not sure how to solve it.

What Gameliving.Die?
Mobs in 30% don't die. They are staying with 0 hp but don't give xp and loot.

That was supposed to be fixed on 6.Feb.2005 but there is another problem that I can't solve right now. Very rare but still could happen.
